Sentara Health Foundation is pleased to announce its acceptance of 2012 Community Recognition Grants applications for funding in 2013. Applications will only be accepted once each year.
Grant application submission deadline is Monday, July 16, 2012, at 5:00 p.m. This is the only date applications will be accepted for 2013 funding.
The Sentara Health Foundation is a division of Sentara Healthcare. The Foundation brings people and resources together to address issues impacting the health and the quality of life in the communities it serves. In addition to addressing the future health care needs of the community, the Foundation Community Recognition Grant Program is designed to provide support and supplemental funding (25% or less of program/project budget) for programs/projects that improve the health status of our community.
Only applications that address one of the Foundation’s REVISED priority areas will be considered for review. They are:
Direct Patient Care – Top Priority
Connection to Care Services – Second Priority
Health Education Programs – Third Priority
