The Sentara Home Care Division is seeking therapists to work in many locations in Southeastern and Northern Virginia to include; Chesapeake, Hampton, Suffolk, Williamsburg, Richmond, Charlottesville or Covington. Physical Therapists will assume responsibility and accountability for a designated group of patients and accurately provides Physical Therapy (PT) evaluations; develops and implements PT treatment plans; provides education and information to patients and caregivers; participates in required discharge planning and complete all required documentation according to policy. Must have a Bachelor or Master Degree in Physical Therapy from an American Physical Therapy Association accredited program and be licensed as a Physical Therapist in the state of Virginia or eligible. Prefer 1 year of experience.
