 Sentara Medical Group is recruiting for a Palliative Care Advanced Practice Clinician to join our Inpatient Hospital Consultation Service at Sentara Obici Hospital. 

The Hospital-based Advanced Practice Clinician (APC) is responsible for providing health care services to individuals, families, and/or groups in the hospital setting, independently and/or under the direction of the attending physician. This position is involved in providing holistic, comprehensive medical care that addresses pain and symptom management, clarification of patient and family goals, relief of suffering and the promotion of quality of life. Health care services include conducting comprehensive health assessments, prescribing/providing treatments/interventions, making referrals, managing acute and stable chronic illnesses, promoting health, and providing disease prevention and management services.   The APC is expected to participate in quality assurance and improvement activities, engage in member- of- the- team activities, use resources (human and material) appropriately, and adhere to Hospital and Medical Staff Bylaws, Rules and Regulations (as applicable). 
 
Essential Responsibilities
 
Inpatient/Facility Services
Provide acute and chronic care for patients under the supervision of the physician as outlined by the scope of hospital and those services that have been supervised, evaluated, and approved by the Board of Medicine.
Performs physical exams and diagnostic procedures under direction of physician.
Orders laboratory and diagnostic studies appropriate to the patient’s complaint.
Reviews and interprets laboratory and diagnostic studies, and ensures the patient receives results and appropriate follow-up.
Formulates a preliminary diagnosis.
Prepares a narrative summary on patient’s condition and treatment plan.
Prescribes appropriate medications, cognizant of both patient restrictions and cost under the direct supervision of a physician.
Initiates therapeutic measures in accordance with accepted protocol under a physician’s direction.
Provides patient evaluation and counseling as indicated.
Confers with professional staff in regard to patient’s physical and mental condition.
 
Night Call Responsibility
May provide night call services to his/her population.
 
Administrative
This role may involve committee work, completion of insurance forms, and completion of other paperwork required by Sentara’s Health Plans.
Works closely with Practice Manager, Director of Operations and Medical Director to manage utilization of health care resources.
Demonstrates commitment to SMG's mission and values, and adheres to policies and rules SMG.
Collects and documents necessary information in patient’s chart in accordance with established policies and procedures.
Maintains accurate records and files as necessary.
Recognizes utilization issues and confers with Medical Director and clinical coordinator.
Attends staff meetings, in-services, and/or continuing education programs on a regular basis.
Aids in controlling department expenses including ancillary services, supply, and personnel utilization.
 
Quality Assurance
Utilizes SMG's CQI program to enhance practice quality.
Participation in quality assurance activities such as chart reviews, committee work, continuing education and in-service for other staff.
Maintains level of proficiency relative to the state of practice of one’s profession.
Offers suggestion to improve jobs, services and functions in work unit and department.
Keeps patient and business information confidential. 
  
Minimum Qualifications 

Education:
Certified Physician’s Assistant or Nurse Practitioner    
Experience: The ideal candidate will have at least five years direct patient care experience in internal medicine, geriatrics, pain management or oncology, with EPEC or ELNEC training, palliative or hospice experience.
Licensure: Virginia licensed Physician’s Assistant or Nurse Practitioner. Expectation of obtaining prescriptive writing certification as available by state law.  
Key Competencies (Knowledge/Skills/Abilities): Has interpersonal skills to manage and develop the staff within his/her center. Has the analytic ability to review internal processes in need of improvement and make suggestions to the Practice Manager, Director of Operations and Medical Director. Has the verbal ability to communicate thoughts and actions to individual patients and center staff.  
Other:  The Hospital-Based Advanced Practice Clinician maintains continuing education credits of at least 30 hours per year and maintains accreditation. Hospital-Based Advanced Practice Clinician acquires special skills as required by the group.   He/She is recognized as a provider and is concerned about his/her patients and who uses science and logic in clinical decision making.   The Hospital-Based Mid-Level Practitioner provides assistance for quality assurance and utilization management activity and for risk management functions. The Hospital-Based Advanced Practice Clinician maintains professionalism and camaraderie with his/her peers while
being recognized as patient friendly. Proficiency in a second language is a plus.
