Sentara Internal Medicine Physicians are quality, board-certified physicians trained in internal (adult) medicine, gynecology, and geriatrics. Transforming Care
Recognized for Quality
We were recently recognized for our patient care model of delivering high quality care based on evidence-based, nationally recognized clinical standards by the National Committee for Quality Assurance. The recognition was for their Physician Practice Connections Program—Patient Centered Medical Home (PPC-PCMH), and we were awarded a Level III NCQA recognition—the highest level awarded.
What Does This Mean For You?
At the heart of patient-centered medical homes is the commitment to work together as a team to provide quality care and a positive experience. Achieving this recognition is just the first step in making real, long-lasting, tangible change to the way care is delivered. But this cannot be achieved without efforts by physicians, nurses, staff AND patients themselves.
We are committed to work together as a team to provide quality care and a positive experience, and we continually look for ways to make long-lasting, tangible changes and improvements to the way care is delivered. But this cannot be achieved without efforts by physicians, nurses, staff AND patients themselves.
