Care Partnering
A Care Partner can be a family member or friend who spends time assisting in your care while you are hospitalized. With the help of our professional staff, your care partner teaches and prepares family members or friends to help assist with care once you are home.
Checkout Time
On most units, checkout time is 11 a.m. If you cannot leave by this time, please notify your nurse so alternate arrangements can be made.
Discharge
Your doctor will decide when you are ready to leave the hospital. A discharge order will be written and you are then free to leave. You may want to make arrangements with a family member or friend to help you when it's time to go home.
Discharge Instructions
Your doctor and your nurse will give you instructions about post-hospital care. If you have questions about your diet, activities or other matters please be sure to discuss them with your physician or nurse.
After You Are Discharged
Sometimes patients need assistance once they are discharged. If you think you may need assistance at home such as nursing care, physical therapy or daily living assistance, our social workers, nurse case managers or physicians can help guide and assist you with post-hospital care programs.
Pain Management
Pain Management is often a combination of therapies, which can include both drug or non-drug interventions to control pain. To assist with pain management, the hospital offers classes and support groups, for more information, dial ext. 4257.
Personal Belongings
Please collect all of your belongings and double-check closets and drawers for any personal items.
