To access you must have an activation code from your physician practice. This code can only be obtained during a scheduled appointment with your physician.

When you activate your MyHealth MyChart account, you have the option of entering your Optima Health member number so that you can use the same username and password to access MyHealth MyChart and MyOptima at Optimahealth.com.
Once logged into MyHealth MyChart you will see a convenient, secure link to MyOptima.
