Sentara Health Management teamed with Sentara Home Care to develop a strategy to deal with members who had chronic asthma conditions. They enrolled select patients in a program to provide education and support for patients and families. SHM created a special capitated arrangement with Home Health which would guarantee success, concentrating spending on prevention rather than treatment. It is the first and only such program in Hampton Roads. ER costs declined by 8 percent, Primary Care provider costs declined by 24 percent and inpatient admissions declined by 46 percent. The total annual savings generated by these efforts are $486,000.
