Sentara Obici Hospital's Community Outreach Program Wins National Award

Sentara Obici Hospital named Program of Excellence for community outreach program.

Suffolk, VA  (February 15, 2011) – Sentara Obici Hospital was awarded a national Jackson Healthcare Charitable Service Award for its community outreach program that offers chronic disease management for people living at 200 percent of the poverty level who have congestive heart failure, diabetes or both diseases.

The award presentation took place Feb. 3 in Atlanta and marked Jackson Healthcare’s inaugural event of naming organizations for the charitable award. The Sentara Obici Hospital Community Health Outreach Program was one of 10 hospital programs that received a $10,000 check in support of the program’s ongoing service. 

The community outreach program, partially funded by Obici Healthcare Foundation, is based upon the premise that individuals with diabetes or heart failure will stick to their medical plan if they have:
Knowledge of their disease process.
Financial resources to purchase medication and foods.
Effective family/social support.

Supported by registered nurses working with the patient’s physician, the nurse assesses patient progress and serves as liaison, providing ongoing education, hope and encouragement to patients to control their chronic disease in their home.

Airfield Branch, 76, credits the care he’s received through the outreach program with saving his life.  “It’s a beautiful, marvelous program that showed me how to take care of myself,” Branch stated.

More than 1,500 patients have been served since the program’s inception in 1998. This winning formula also caught the attention of the Jackson Healthcare National Hospital Charitable Award as a terrific example of a program that has real impact.

Over 100 hospital programs were evaluated on five criteria: community impact, innovation, collaboration, transferability and best practices.
