New Service Eases Transition from Hospital to Home

Norfolk, VA (February 2012)– A smiling face and a bright new retail space mark the launch of Sentara To Home, a pilot program now underway with cardiac inpatients at Sentara Norfolk General Hospital.

Sentara To Home is a service initiative bringing convenience and quality to the bedside by delivering prescription medications and portable home medical supplies to patients’ rooms prior to discharge. If patients desire, the program’s Transition Specialists will also work with patients’ physicians to arrange follow-up office visits and with Sentara Home Care Services for delivery of larger items.

“We want to help patients and their families concentrate on getting better,” says Tim Jennings, vice president of Pharmacy Services for Sentara Healthcare.

“We hope Sentara To Home will reduce the running around that family members have to do for prescriptions and supplies by putting those items in their hands.”

New retail store offers variety, convenience

Just off the Kaufman Lobby corridor at Sentara Norfolk General Hospital, the Sentara To Home retail store is now open, offering outpatient prescriptions and over-the-counter medications, plus a broad variety of home care supplies to make recovery from a hospital stay easier.  They range from an ingenious pouring device for gallon milk jugs to motorized up-and-down easy chairs and comfort wraps that heat up in the microwave.

“Going home is part of the patient experience,” adds Tim Jennings.  Sentara To Home will help us maintain our service to patients throughout the continuum of care.”

Still in a trial phase at Sentara Norfolk General, Sentara To Home is expected to eventually expand to serve all Sentara hospital patients.
