NV: Classes & Events

Being hospitalized can be a difficult, confusing time. Sorting out physical and emotional problems due to an illness and making sense of information and choices can be overwhelming.

Sentara Northern Virginia Medical Center’s Care Coordination Department is available to help hospitalized patients and their families. We can meet with you to discuss your concerns and guide you in finding solutions. Our Care Coordinators have extensive experience and skills in assisting people within the medical system.

How care coordinators can help you
You may need continuing care and special assistance at discharge; someone from our department can work with you to plan for this possibility. We want to make sure you leave the hospital with the knowledge and resources you need to feel confident about managing your healthcare needs.

Our services are available at no charge to patients in the hospital. If we can be of service to you and your family, please ask your doctor, nurse, or any hospital staff to make a referral.

Care coordination Services
We offer counseling, problem solving, planning and information services to help patients and their families while you are in the hospital.

Counseling and Problem-Solving Services to help you

  • Identify and discuss emotional and practical issues that may affect how well you cope with your illness.
  • Explore your choices and options so you can develop a plan to manage your healthcare needs that works for you.
  • Consider positive ways to deal with changes caused by illness or disability.
  • Talk about and understand end-of-life choices, decision-making, and resources including hospice care, living wills/advance directives, and grief counseling.
  • Make sure your concerns are heard by mediating communications between you, your family, and your hospital team.
  • Support you in understanding your rights as a patient and advocate on your behalf.

Planning and Information Services to help you and your family

  • Work with your doctor and other members of your healthcare team to identify what services you might need in the hospital and when you go home.
  • Arrange for as rehabilitation, a skilled or intermediate nursing facility, or assisted living after your stay in the hospital.
