Completing the New Hire Process
New Hire PaperworkCongratulations on your new position. We are glad you are joining the Sentara family. Prior to the first day of orientation or first day of work, you must complete new hire paperwork. We provide many of the forms here so you can complete them at your convenience.
Form Technical Details:
You must have Adobe Reader to use these forms. Adobe Reader is free software that lets users view, fill and print filled in Adobe PDF files on all major computer platforms. Some of the forms are provided as "fillable" PDF files for your convenience. You must have at least Adobe Acrobat Reader 4.0 installed on your computer to view, complete and print fillable forms. For "Fillable" PDF files: Acrobat Reader does not allow you to save what you key into the form, nor can you email the completed form - you should plan on completing and immediately printing completed forms. For some people, it may be easier to print and complete all of the forms by hand. You may do what works best for you.
Bring Forms to Orientation It is important that as much new hire paperwork is completed prior to your attendance at orientation. It saves you and the other orientation attendees valuable time.
Please turn in all New Hire Paperwork at Day 1 of your scheduled Orientation at our Corporate office (see item below for additional detail). If you fail to complete and return your new hire paperwork on Day 1 of Orientation, you will not receive your employee ID badge AND cannot attend Day 2 of your Orientation.
If you have questions regarding the new hire forms, please contact your HR Recruiter or stop by one of our Employment Centers for assistance prior to your Orientation. Individuals from the HR Staff will also be present at Day 1 Orientation to answer limited questions during the beginning of each of the sessions.

Form Details: In compliance with federal law, Sentara Healthcare collects responses solely for reporting purposes. This information will be kept separate from your employment application and personnel file. Federal law prohibits unlawful discrimination on the basis of race, color, sex, age, national origin, religion or disability.

Form Details: This information is required to verify accuracy of previously provided information and to ensure accuracy of our employee database. This is also required for IRS reporting, maintenance of employee records and data, and for processing of benefits information.
Form 3: State Tax Forms (Virginia and North Carolina, if applicable) (PDF Files)

Form 5: I-9 and Instructions for I-9- Instructions (PDF File) - I-9 (Fillable PDF File)

Form Details: All new and rehired Sentara employees must complete the federal Employment Eligibility Verification Form (I-9) and show proof of employment eligibility within three business days of the date of employment. Employees must present original documents to their employer; no photocopies can be accepted.
