Patient Rights and ResponsibilitiesSentara Obici Hospital adheres to the guidelines established by the Joint Commission on Accreditation of Healthcare Organizations for the Rights and Responsibilities of Patients. In order to give you the best care possible, we need you to follow the list of rights and responsibilities.
• You have the right to know by name the physician,nurses and staff members responsible for your care.
• You have the right to hear from your primary physician, in language you understand, your diagnosis, the treatment prescribed for you, the prognosis of your illness, and any instruction required for follow-up care.
• You have the right to talk openly with your physician and to know the reason you are given various tests and treatments and who the persons are giving them to you.
• You have the right to know the general nature and inherent risk of any procedure or treatment prescribed for you and the right to refuse to participate in medical training programs and research projects.
• You have the right to change your mind about any procedure for which you have given consent, and the right to cross out or refuse to sign any part of the consent form if you feel everything has not been explained to your satisfaction. You also have the right to refuse treatment and to be informed of the medical consequences of this action.
• You have the right and responsibility to notify the nurse if you would like to limit those persons who visit you and to expect your personal privacy to be respected to the fullest extent consistent with the care prescribed for you. In turn, you have the responsibility to respect the privacy of your roommate.
• You have the right to expect all communications and other records pertaining to your care, including the source of payment for treatment, to be kept confidential.
• You have the right to request a consultation or second opinion from another physician, to change physicians, or change hospitals.
• You have the right to examine your hospital bill and to receive an explanation of it.
• You have the right to have impartial access to the medical resources of the hospital indicated for your care without regard to race, color, creed, national origin, age, sex, handicap, or source of payment.
• You have the right to have your pain thoroughly assessed and promptly treated.
• You have the right to have your pain reassessed regularly and your treatment adjusted if your pain has not been eased.
• You have the right to Personal Safety and to expect safety standards to be followed within the facility, and to be free from all forms of abuse.
• You have the right to be free from restraints used in behavior management and provision of acute medical and surgical care unless clinically necessary.
• You have the responsibility to know and follow hospital rules and regulations. You are also responsible for cooperating and following the care prescribed or recommended for you by your physician, nurses, or allied health personnel.
• You have the responsibility to notify your physician or head nurse if you do not understand your diagnosis, treatment or prognosis.
• You have the responsibility to accept your financial obligations associated with your care.
• You have the responsibility to advise your nurse, physician, or caregiver of any dissatisfaction you may have in regard to your care at the hospital.
• You have the responsibility to work with your doctor and nurse to make a pain relief plan.
• You have the responsibility to tell the doctor or nurse about any pain that will not go away.
• You have the responsibility to be considerate of the rights of other patients and hospital personnel and to assist in the control of noise, smoking, and the number of visitors you receive.
