An Exciting New Alternative to Nursing Home Care
Sentara Senior Community Care recognizes that older people often have medical problems that last for long periods of time and require services from several different sources. Of great concern is the fact that, as many people age, they become increasingly isolated from the support, companionship and resources of the community at large. Sentara Senior Community Care is that important link; assuring participants receive the care they need, when they need it.
About Sentara Senior Community Care
Senior Community Care is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community. It is an alternative to nursing home eligible, but who would prefer to remain at home with services coordinated by Sentara.
The first of its kind in Virginia, the program is a prepaid health plan that provides total care for participants, including comprehensive medical and rehabilitative services, in-home services and transportation. The program is offered in Norfolk, Virginia Beach, Chesapeake, and Portsmouth.
The program is modeled after a national replication project called PACE, or Program of All-Inclusive Care for the Elderly, and will be granted special waivers from the Medicare and Medicaid programs. The program is operated by Sentara Life Care Corporation (SLCC), Sentara's long-term care division.
Assistance is available from doctors, nurses and a full team of health care professionals all working toward a common goal: to help participants remain active in their families and community. The result is healthier participants who spend less time in the hospital and often never need nursing home placement.
Sentara Senior Community Care Services
Sentara Senior Community Care provides comprehensive medical care to minimize health problems and maximize levels of independence.
Most services are provided at our Day Health Centers, which combine the services of an adult day care center with a primary care physician's office.
Services such as nursing, social work, physical, occupational and speech therapy, and nutritional and personal care are all provided in these community-based centers.
There, participants enjoy meals, snacks, and recreational and social activities. Transportation is also provided. All care is monitored and coordinated by a Personal Care Team.
Sentara Senior Community Care is different from any other kind of health care program. It offers participants a comprehensive health care program that is more personalized than ever. Sentara Senior Community Care provides a full range of services 24 hours a day, 7 days a week, 365 days a year.
