Sentara Medical Group Inpatient Physician Team

A Qualified Inpatient Physician will be at your side.
Your Primary Care Physician has selected a special team of dedicated Sentara Medical Group physicians to oversee your care while you are a hospital patient. These Sentara Medical Group physicians are based full-time at Sentara Norfolk General Hospital and Sentara Leigh Hospital.

Your Physician-Team provides 24-hour Hospital Coverage.
Continuity of care is provided by the Physician Team since they consult with specialists, discuss diagnostic tests results and answer questions about your care. These physicians have the opportunity to build strong relationships with hospital staff, discharge planners, specialty consultants and outpatient physicians.

Communication is the key.
While you are in the hospital, one member of the Inpatient Physician Team maintains overall responsibility for your care. There is, however, constant contact between the Inpatient Physician and your Primary Care Physician regarding your status and ongoing care.

Expert care, when and where you need it.
If you need to be hospitalized, your Primary Care Physician will contact the on-call Inpatient Physician Team member when you are being sent to the hospital for a work-up and possible admission. Should you need to go directly to the Emergency Room and be admitted, a Team member will place a call to your Primary Care Physician to let your doctor know you have been admitted to the hospital and to discuss your care.

Nurse Coordinator coordinates community resources.
The Inpatient Physician Team includes a community resources nurse coordinator to assist in communications with hospital staff, nursing homes, and home health providers. In addition, the nurse coordinator can help with family matters, educational needs and coordination of insurance related issues.

At Discharge, your personal physician receives immediate communications from the Inpatient Team.
When you are ready to go home, the Sentara Medical Group Inpatient Physician Team will fax a one-page summary discharge sheet of important information to your Primary Care Physician - instant communications to ensure continuity of your follow-up care. This communication quickly provides your physician with a discharge diagnosis, lab results, current medications, and a summary of all pertinent information. A full hospital discharge report will be sent to your Primary Care Physician within 48 hours.
