MyChart
 MyChart is a web-based tool that provides secure and convenient access to your physician and personal electronic medical record.
If your doctor is in a participating physician practice that offers MyChart, you can access this online service that provides convenient access to your medical record.
To access you must have an activation code from your physician practice. This code can only be obtained during a scheduled appointment with your physician.
MySentaraHere's where you have convenient access to selected health care services like paying a hospital bill or downloading common patient forms.
MyOptimaWhen you activate your MyHealth MyChart account, you have the option of entering your Optima Health member number so that you can use the same username and password to access MyHealth MyChart and MyOptima at Optimahealth.com.
Once logged into MyHealth MyChart you will see a convenient, secure link to MyOptima.
