What is MyChart?

MyChart is a web-based tool that provides secure and convenient access to your physician and personal electronic medical record. This electronic medical record is also accessible between Sentara hospitals, physician practices and other healthcare sites over a secure network.


What is MyHealth MyChart?

MyHealth MyChart is a secure internet portal designed for patients and individuals who use Sentara services. MyHealth MyChart provides access to applications and other features which allow you to be more involved in your healthcare. MyChart is an application that patients access using MyHealth MyChart.



How do I get access to MyChart?


If your doctor is in a participating physician practice that offers MyChart, you can access this online service that provides convenient access to your medical record. To access you must have an activation code from your physician practice. This code can only be obtained during a scheduled appointment with your physician.

My activation code for MyChart doesn’t work, what should I do?

For your security, your activation code expires after 30 days. Once you use your activation code, it is no longer valid. If you allow your activation code to expire, contact MyChart MyHealth at 1-800-SENTARA (1-800-736-8272 | Mon-Fri | 8 a.m. - 6 p.m). You may also obtain a new activation code from your physician practice by presenting a form of identification to confirm your identity and requesting MyChart.


Can I create my own password and change it when I want?


Yes. Please keep your password confidential, and do not share it with anyone else. You may change your MyHealth MyChart password at any time by clicking on “Preferences” after you login to MyHealth MyChart. Your password is not maintained in a readable format, therefore, if you believe someone else knows your password please change it.


What should I do if I get a "login unsuccessful" message from MyHealth MyChart when accessing MyChart?

ake sure your “Caps Lock” key is not on. The password field is case sensitive. Make sure you do not enter extra spaces in either the MyHealth MyChart ID or password boxes and try again. If you have forgotten your MyHealth MyChart ID or password, click on "Password Help" or “Login ID Help” links.
