Advance Directive

Sentara Healthcare and the Sentara Center for Healthcare Ethics are offering the opportunity for the community to register their Advance Directive with our national Advance Directive Registry. Where ever you go in the United States your Advance Directive will be accessible by health care professionals when needed to guide your medical care if you are terminally ill and unable to communicate your wishes. For more information on writing an Advanced Directive and definitions of commons words used in them, refer to "Writing an Advanced Directive."
