Home Care

Sentara offers comprehensive home care services for those patients who need continuing care after returning home. These innovative and progressive home care programs combine the benefits of interpersonal family relationships with optimal medical and nursing treatment and support. Home care services can provide all levels of care, from the 'high-tech' to the routine. This includes such services as dressing changes, medication teaching and administration, gait training and disease management.

Depending on the patient's needs, home health care may involve registered nurses, personal health aides, and/or a licensed clinical social worker, who can address emotional and familial issues and give the patient and family information about additional community resources. Sentara Home Care Services also offers specialty programs such as the Mental Health Home Care Program, Asthma Home Care Program, Hospice, Private Duty and Pediatric Services.

Specific neurological services include:

 Skilled nursing for assessment of neurological status, progression of disease, medication teaching, symptom assessment related to medication usage, and nutritional assessment.

 Physical and occupational therapy for gait training related to decreased motor tasks, proprioception problems, assistance with activities of daily living.

 Speech therapy services for cognitive function in learning, memory and abstract thinking.

 Dysphagia therapy for swallowing disorders as a result of a neurological disease or trauma.

 Home health aide services for short- or long-term personal care, respite for caregivers or rehabilitation of patients with neurological disorders.

 Homemaker services to assist the patient with a neurological disorder with general housekeeping, food shopping and meal preparation (with prior authorization from insurance carrier).

Home Medical Equipment
Sentara Home Medical Equipment (HME) rents and/or sells equipment and related services that help the patient meet the therapeutic goals to recover within the home environment. HME will deliver, set up and maintain the necessary equipment (as stipulated by a physician's prescription), and educate and train the patient or caregiver on proper use. A complete range of medical equipment, supplies and accessories are available, including hospital beds, wheelchairs, biliblankets, and ostomy and diabetic supplies. Sentara Respiratory Services can set up and deliver (portable) oxygen, liquid gas oxygen, CPAPs/BiPAPs, nebulizers and apnea monitors. All services are available 24 hours a day, seven days a week, serving patients throughout southeastern Virginia and northeastern North Carolina.
