Have you ever wanted to express your opinion or make a suggestion regarding your home care or hospice experience, but didn’t know how? If the answer to this question is yes, then take advantage of this opportunity to get involved with the Patient & Family Advisory Council (PFAC) with either Sentara Home Care Services or Sentara Hospice Services.
The Sentara patient-centered care philosophy recognizes the perspectives of patients and their families as valuable resources! We encourage the involvement of patients and families in shaping our home care and hospice services. Sentara Home Care and Hospice has built a community partnership with the PFAC developing and nurturing an ongoing relationship. Established in 2013, the PFAC is recruiting for new members.

PFAC members of Sentara Home Care Services and Sentara Hospice Services commit their time, efforts and talents to serve in a consulting capacity for a variety of home care or hospice issues and programs. Working together, we support Sentara’s mission to "improve health every day."

expresses interest in the patient-care process
