Sentara offers comprehensive home care services for those patients who need continuing care after returning home. This includes such services as dressing changes, medication teaching and administration, gait training and disease management.
Depending on the patient's needs, home health care may involve registered nurses, personal health aides, and/or a licensed clinical social worker, who can address emotional and familial issues and give the patient and family information about additional community resources.
