An Exciting New Alternative to Nursing Home Care
Sentara Pace with two locations, Sentara PACE Churchland, located in Portsmouth and Sentara PACE – Virginia Beach recognizes that older people often have medical problems that last for long periods of time and require services from several different sources. Of great concern is the fact that, as many people age, they become increasingly isolated from the support, companionship and resources of the community at large. Sentara PACE is that important link; assuring participants receive the care they need, when they need it.


About Sentara PACE - Churchland and Sentara PACE - Virginia Beach
This program is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community. It is an alternative to nursing facility eligible, but who would prefer to remain at home with services coordinated by Sentara.
The program is one that provides total care for participants, including comprehensive medical and rehabilitative services, in-home services and transportation. The program is offered in Norfolk, Virginia Beach, Chesapeake, Suffolk and Portsmouth.
The program is modeled after a national replication project called PACE, or Program of All-Inclusive Care for the Elderly and is an approved Medicare and Medicaid provider. The program is operated by Sentara Life Care Corporation (SLCC), Sentara's long-term care division.
Assistance is available from doctors, nurses and a full team of health care professionals all working toward a common goal: to help participants remain active in their families and community. The result is healthier participants who spend less time in the hospital and often never need nursing home placement.
The PACE program is an all inclusive healthcare program for qualified participants. PACE participants may be fully and personally liable for costs of unauthorized or out-of-PACE program agreement services.
