How do I get a My Albemarle Medical Record account?
You may self-enroll by following these directions, as long as you have an e-mail address on file in your medical record at the hospital and know your Medical Record number. If you do not have an email address on file in your medical record and do not know your medical record number, you can come to the registration department at the hospital and we will be happy to assist you in getting set-up.
Please note: If you share an email address with a person that already has a My Albemarle Medical record account using the shared e-mail, you will not be able to enroll using the same e-mail. You will need to use a separate e-mail address to enroll. (Patient will receive a message “you appear to already be enrolled” if enrolling using the same e-mail as another person)
I lost my activation pamphlet, who should I contact to get another copy?
Medical Records or registration on the first floor of the hospital will be able to provide you the information.
I forgot my login ID, how do I find out what it is?
You can contact the My Albemarle Medical Record at 252-384-4475 and select option 4 and the service desk can reissue you a new login. The new temporary login information will be sent to the email you used when signing up for the portal. For security reasons and your protection new log-in information can not be given verbally over the phone.
How do I change my login id?
Please contact us between 8 a.m. – 5 p.m. at 252-384-4475.
I forgot my password, what do I do?
- You will need to use the Forgot Password feature
- Enter in your Logon Id
- Enter your Email Address
- A temporary password will be sent to your email
How do I change my password?
- Login to your My Albemarle Medical Record and select Preferences
- Select Change Password
- Enter your Old Password and then your new password twice
- Click Submit
- You will need to log off and then log back in before continuing.
Where does my health information in My Albemarle Medical Record come from?
All of your personal medical information is from your Electronic Health Record at Sentara Albemarle Medical Center. That way, you get the most accurate, up-to-date information possible.
What should I do to ensure that my health information is secured when using My Albemarle Medical Record?
My Albemarle Medical Record is a secure application. It is also important that when you exit My Albemarle Medical Record, you also logoff of the My Albemarle Medical Record website and close all browsers that you have open. This will ensure that your clinical data remains secure.
Where is patient data stored?
No data is stored on the web server. All data is retrieved dynamically from the application file servers as pages are accessed.
Is my information safe?
Yes. My Albemarle Medical Record passwords are encrypted and page links are rewritten so that they cannot be copied and pasted. You and only those you authorize can access your medical record. Also, a timeout feature protects your information by logging you off automatically if you leave the My Albemarle Medical Record open.
When I click on the My Albemarle Medical Record link nothing shows up or I receive an error message.
This may be due to browser settings on your personal computer. Make sure that cookies are enabled on your browser. If you are receiving an error message please contact 252-384-4475 to log a technical issue.
Am I restricted to using one web browser?
My Albemarle Medical Record is compatible with most web browsers and has been tested with Safari, Chrome, Internet Explorer, and Firefox.
Can my family access My Albemarle Medical Record?
Yes, you can give family members, such as parents, children or healthcare proxies, access to your My Albemarle Medical Record. Once you have filled out the appropriate form, please bring it to hospital registration or medical records along with a photo ID. Proxy access will be granted within 24 hours.
