Please use this form to submit requests for bio cards. A member of the Sentara Medical Group Marketing Team will review your request and get back to you as soon as possible.

Please note that all sections need to be filled in completely, “N/A” or “No Information” are not acceptable. The SMG Marketing Team will not begin creating the bio card until all of the information is filled out.
