Heart Check Program at Sentara RMH Medical Center

Are you aware of heart risk?  Have you been challenged to make lifestyle changes - but aren't sure how to start? Are you having a hard time reaching health goals?

Heart Check is a self-pay, self-referral $50 cardiovascular risk assessment with a focus on minimization of risk appointment offered by a cardiac nurse who is also a Duke Certified Integrative Health Coach.

The Heart Check program provides a unique dual approach of cardiovascular disease risk assessments and meaningful health coaching. Heart Check provides awareness of risk and personal health planning and discount advanced screenings based on your results, vision and values. A copy of your risk profile, along with lab results, is provided to you and can be forwarded to your healthcare provider.

Personal awareness and value based coaching can help you by providing forward-focused, personalized health planning that uses various stages of learning and change. The nurse coach uses integrative health tools to nurture, motivate and promote learning.

To schedule, call Sentara RMH Central Scheduling at 540-689-6000. Option 2 (twice). You'll be sent a brief assessment tool and blood work form in the mail to complete prior to your appointment. Instructions about where to go for your blood work is provided on this form. 

If you have recent blood work results in hand (Total Lipid panel and glucose) and want to use them for your Heart Check, you may fax them to 540-689-6801 prior to your appointment.
