- On August 8, 2025, TPMG Behavioral Health will close and a new Sentara Medical Group office will open at the same location on Rock Landing Drive. The new practice will be named Sentara Behavioral Health Specialists.
- We are excited about the opportunity to continue providing patients with high-quality, comprehensive care under a new practice name.
- Although the transition to Sentara Medical Group will take effect on August 8, the office will be temporarily closed for in-person visits due to scheduled construction. We will reopen for patient appointments in the office on August 25.
- During the transition period, patients can still call the office for medication refills and to schedule future appointments.
- While our practice name is changing, our office hours, address and phone number will remain the same.
- As part of this change, we will be transitioning our current practice pages on the TPMG website to the Sentara Health website. Starting on August 8, you can find information about our practice and providers on sentara.com.
- You may continue to use the NextGen patient portal for any needs related to your other TPMG providers. However, after the transition, you will need to sign up for MyChart to access your Sentara records. To get started, visit sentara.com/MyChart and register as a new user. If you are already a Sentara patient and have a MyChart account, simply log in to access your medical records.
- If you have an upcoming appointment, please contact us at 757-595-3900 on or after August 20 so that we can ensure it is properly scheduled. We’re committed to making this transition as smooth as possible and look forward to continuing your care.
- For future appointments, please continue to call 757-595-3900 and a member of our care team will gladly assist you.
- During the initial transition period, you may receive bills from TPMG Behavioral Health for services previously delivered. For services after August 25, bills will come from Sentara Medical Group.
- We will continue to accept most insurance plans offered by major insurers. For specific questions about your coverage, please contact your insurance provider directly.

Can I still access my medical records through the NextGen patient portal after the transition?
Yes. You will continue to have access to your medical records from TPMG through the NextGen patient portal. Going forward, any new records and information related to your care with Sentara Behavioral Health Specialists will be available through Sentara MyChart. To get started, visit sentara.com/MyChart and register as a new user. If you already have a Sentara account, you can simply log in to access your Sentara records.
