How to schedule and prepare for an appointment
Our team is committed to working with your physician. Your physician office should submit medication orders, history, physical and additional required documentation to our fax: 757-452-5753, then call the Outpatient Infusion Center at 757-388-2066 to complete the referral process.
During the referral process, the team will gather demographics, insurance information, and treatment information to schedule your first appointment.
Appointments are made 2-3 weeks out due to the need for approved prior authorization from your insurance carrier.
Insurance authorization and verifications should be done by the referring physician office, but we have a team that can complete as well.
Any subsequent appointments are scheduled through the Outpatient Infusion Center Staff.
