  • How do I log in to the Sentara mobile app?

    Once you download the Sentara mobile app, you can login using your Sentara MyChart username and password.

    If you do not have an account, you can sign-up here or follow the steps outlined in the next question below.

  • How do I sign up for, or activate, my account?

    Step 1: Request an Activation Code

    • If you have recently completed or scheduled an appointment with a Sentara provider, you may have received an activation code from them; or, it may be printed on the last page of your recent visit summary or discharge instructions.
    • You may also request an activate code here; or open the Sentara app, click “Sign Up”, then select “Request an Activation Code”.
    • You will be asked to fill in your personal information, including your address, date of birth, social security number and email address.
    • Then, you will be asked three questions to verify your identity.
    • When you’re done, check your email for your activation code.

    Step 2: Use Your Activation Code to Complete Sign Up

    • Once you have your activation code, click here. Or, open the Sentara app, click “Sign Up”, then select “Activate My Account”, then click “Confirm & Submit”.
    • Read the terms & conditions, agree to the terms, and then click “Next” or ”Next Step”.
    • Enter your 15-digit activation code, and re-enter your personal information.
    • Next, create your username and password.
    • Finally, you’ll select three security questions to answer in case you forget your password.
    • You will receive an email when your account set up is complete.

  • How do I choose a username and password?

    Your username must:

    • must be between 5 and 20 characters
    • begin with a letter
    • include only letters, numbers, and underscore
    • cannot include spaces or special characters
    • is not case sensitive.

    Once selected, your username cannot be changed.

    Your password must:

    • be at least eight characters
    • contain one uppercase letter
    • contain one lowercase letter
    • contain one number or symbol
    • be different from your Sentara or Sentara MyChart username / ID.

    Please keep in mind that only you will know your username and password.

  • I forgot my password. How do I update my password?
    • On the app home screen, below the "Sign In" or "Log In" button, click "Forgot Password." Enter your username, and then click "Submit."

    • You will receive an email with a link to reset your password. If you do not receive an email, request assistance here.

    • Use the link to answer the password reset question you created when you first signed up for your Sentara MyChart account.

    • Select a new password that meets the noted security requirements.
  • I forgot my username. How do I access or update my username?
    • On the app home screen, below the "Sign In" or "Log In" button, click "Forgot Username." Enter your information, and then click "Submit." The information you provide must match what is on file.

    • You will receive an email containing your login ID. If you do not receive an email, request assistance here.

    • NOTE: Your username is permanent and cannot be changed.
  • How do I get an activation code?

    If you have recently visited a Sentara provider, you may already have an activation code printed on the last page of your visit summary or discharge instructions. You may also request an activation code here.

  • Why am I receiving an error when I try to activate my Sentara account?

    Please make sure that your full valid social security number and date of birth are on file in your Sentara medical record. You may do this by contacting your Sentara provider's office or any one of our outpatient registration departments at the hospital. You may also need to validate that you have entered your activation code correctly. If you have checked both items above and still encounter an error, please contact us.

  • Will I still be able to access my medical records through MyChart?

    Yes, you will still have access to your medical records through MyChart. You will use the same username and password to log in on Sentara.com and the Sentara app.

    However, the Sentara mobile app offers the same functionality, as well as new features, that may not be available through MyChart.
