Patient Guide

Thank you for your interest in Sentara Medical Group. Please select from the following options for more information and patient forms that will help us provide you the best care.

  • Becoming a Patient

To get help finding a doctor or to schedule an appointment, please call 703-523-8880.

Appointments with our specialists may require a referral, however, our team will assist you with that process.

    Request an appointment online

Don’t have time to call, but want us to help with the scheduling process? Fill out our online request form and a member of the Sentara Medical Group Care Connection team will follow up with you on the next business day.

  • Patient Forms

In an effort to make your appointment check-in process smoother and more efficient, here you can find a number of forms that you may fill out in advance of your appointment.

If you are a new patient for a Sentara Medical Group practice, you can download the appropriate form below (Adobe Acrobat Reader required). Print and complete the form, and bring it with you to your first visit.

Authorization to Disclose or Release Health Information
If you want to have a medical record transferred from another doctor’s office to your new Sentara Medical Group office, please complete this form. Likewise, this form can also be used to authorize someone other than you to have access to information about your healthcare status on treatment. Just print and complete the form and bring it with you to your next appointment.

For questions about these forms, please call your Sentara Medical Group practice.

  • Billing and Insurance

Sentara Medical Group is dedicated to ensuring you understand the billing process and the resources available to you. We offer multiple ways to pay your bill - online, through the mail, on the phone, or in person at the Sentara Medical Group office.

  • Sentara MyChart

We encourage all of our patients to sign up for Sentara MyChart. It’s a great way to communicate with your care teams, schedule an appointment, request a prescription refill and view test or lab results – all on your own time. It also keeps you up-to-date on when you’re due for routine vaccinations or other preventative screenings so that you can be a more active – and proactive – part of your healthcare team.

  • Patient Resources

One of the ways we deliver quality care is by helping connect you to additional information and resources even in between your visits to our office. Below you will find links to online resources, as well as information about neurology and a wide-range of disease-specific support groups offered at Sentara.
