Becoming a Patient
Patients are generally referred to our practice because they have an abnormal symptom or screening test or because they have already been diagnosed with cancer. At your first appointment, we conduct a physical exam and discuss your personal circumstances, including your health history. We may ask you to bring copies of X-rays, CTs or MRIs with you at the time of your first visit. Our goal is to ensure we have all the information possible to recommend the next steps in your care.
During your evaluation, you may be referred to another specialist who is trained to provide a specialized diagnostic test, treatment or procedure. Examples of specialists include radiation oncology and medical oncology.
In addition, many patients are referred to our patient navigator. Patient Navigators are your resource contacts, who will guide you from the initial diagnosis through treatment. They can help you locate a support group, review treatment options and discuss the care you’ll need before and after treatment. To schedule an appointment, please contact us at 703-523-8860. Authorization to Disclose or Release Health Information
If you want to have a medical record transferred from another doctor’s office to your new Sentara Medical Group office, please complete this form. Likewise, this form can also be used to authorize someone other than you to have access to information about your healthcare status on treatment. Just print and complete the form and bring it with you to your next appointment.

Billing and Insurance
Sentara Medical Group is dedicated to ensuring you understand the billing process and the resources available to you. We offer multiple ways to pay your bill - online, through the mail, on the phone, or in person at the Sentara Medical Group office.
We encourage all of our patients to sign up for Sentara MyChart. It’s a great way to communicate with your care teams, schedule an appointment, request a prescription refill and view test or lab results – all on your own time. It also keeps you up-to-date on when you’re due for routine vaccinations or other preventative screenings so that you can be a more active – and proactive – part of your healthcare team.
