About the Mobile Clinic
The Sentara Family Health Connection Mobile Clinic program started in 1996 as a way to serve residents in Eastern Prince William and Northern Stafford Counties who wouldn’t otherwise receive care. Over time, the program has grown to include two mobile clinics. These clinics provide primary care health services to underserved patients throughout the area. The Family Health Connection serves patients with no insurance, who are 200% and below of the federal poverty level and who reside in eastern Prince William and north Stafford counties.
The clinics are staffed with a Certified Nurse Practitioner and a Medical Assistant. They travel to various sites throughout the week to provide primary care services to those who need and qualify for services.
Services include preventative health maintenance, physicals, well-woman exams, cancer screenings, chronic illness management, sick visits, pregnancy testing, medication assistance and referrals to specialists.
The program is made possible through the generous support of The Sentara Northern Virginia Medical Center Auxiliary, the Potomac Health Foundation and a number of health partners throughout our community.
The Sentara Family Health Connection Mobile Clinic care team includes nurse practitioners certified by the American Academy of Nurse Practitioners.
