Advance Care Planning Workshop

Join us for this free event to learn more about Advance Care Planning (ACP). By the end of the program, you will understand the importance of having an ACP on file with the registry. There will be a trained facilitator on site to assist if you have questions about the ACP paperwork.

Additional directions:

This class will be held at 920 E High Street in Suite 103, at the Community Health and Outreach Center. There is a parking garage behind the building with entrances on E High Street and 10th Street. The Community Health and Outreach Center is located on the same floor as the E High Street parking garage entrance.
