All information contained in the database is secure and cannot be altered by outside parties.

On the screen titled "Verification Lookup Portal" enter all or part of the last name and/or the first name. If you require your name, title, and organization to be listed on the letter, enter the information.
Click the box acknowledging that you have permission from the practitioner to look up their information, and then click on the "Search" button at the bottom of the screen. All names meeting the criteria entered will appear.
Select the name of the individual for whom a query is requested, and a letter will appear.
We will no longer provide individual verification letters for providers who can be verified using the website. Requests received for such will be returned to the requesting organization.
