We are here to help
Every person’s health journey is unique, and so are their home care needs. At Sentara Home Care Services, our experienced intake specialists provide an initial phone evaluation at no cost. During this call, we’ll talk with you about your health needs, home environment, and personal goals.
Admissions and referrals
Home care services can be requested by a patient, physician, family member, or friend. A physician may prescribe home care as follow-up after surgery, recovery from a serious illness, or to provide comfort-focused care for someone with a terminal condition.
You will need to coordinate skilled medical home services with a physician to get started. Our intake specialists can guide you through this process if you do not already have a referral.
Eligibility for home care is based on medical need and may be covered by insurance, including Medicare or Medicaid, depending on the situation. Patients can be eligible for home care services regardless of prior hospitalization. Learn more about how to determine if it’s time for skilled medical home care services.
