How to schedule and prepare for an appointment

Our team is committed to working with your physician. Your physician office should submit medication orders, history, physical and additional required documentation to our fax: 757-452-5753, then call the Outpatient Infusion Center at 757-388-0366 to complete the referral process.

During the referral process, the team will gather demographics, insurance information, and treatment information to schedule your first appointment.

Appointments are made 2-3 weeks out due to the need for approved prior authorization from your insurance carrier.

Insurance authorization and verifications should be done by the referring physician office, but we have a team that can complete as well.

Any subsequent appointments are scheduled through the Outpatient Infusion Center Staff.

  • Appointments are available seven days a week (weekends only IV antibiotics) and must be pre-scheduled.

Physician instructions

Please ensure that you follow/complete the specific instructions provided by your Physician before your infusion, such as medications you may take, what to eat, and/or ordered blood work.

Gather your information

Assemble the important documents you'll need and keep it in a folder. The documents you will need include:

  • Current form of identification
  • Insurance card(s): Please ensure you are aware of your benefits and any co-pays you may incur
  • Medical history
  • Medication list

Arrange for transportation

We recommend arranging for transportation as you may have side effects like weakness, sleepiness or feeling dizzy. Until you know for certain how you will respond to your treatment, have a way to get home other than driving yourself.

  • Many insurance plans cover the cost of a rideshare service (like Uber or Lyft). Check with your insurance plan.
